Polish fans allegedly chanted racist slogans and set off flares at Stadio Renato Dall’Ara during Poland’s Friday clash with Italy, which ended in a 1-1 tie.
UEFA’s ethics committee is to hear the case on September 21, according to Football Italia.
Poland is among the first countries facing charges involving the UEFA Nations League, a new competitive football format designed to replace most international friendlies in Europe.
Disciplinary proceedings have also been opened against Ukraine and the Czech Republic over alleged crowd unrest, the Associated Press has reported.
